FormsPLUS 6.12 Download

FormsPLUS contains nearly 200 forms and exhibits from the Local Assistance Procedures Manual (LAPM) and Local Assistance Program Guidelines (LAPG).

This new version (6.12) of FormsPLUS is issued for new LPP 09-01, while minor changes were also made for some forms of the new Race Conscious DBE Program. Form 12-D is also revised (updated on June 23).

Local Assistance Program Guidelines and Procedures Manual updates are issued by means of Local Programs Procedures (LPPs), from Caltrans Division of Local Assistance, Office of Procedures Development and Training. LPPs are distributed by mail in hard copy form to manual subscribers and are also posted on the Local Assistance web site along with updated versions of all affected chapters.

FormsPLUS 6.12 modified eight exhibits based on LPP 09-01. A new feature "Fast Import" is introduced to expedite the importing process from existing forms. This function is only available for these to use the following patch. For further instruction, please contact FormsPLUS@dot.ca.gov
