PLEASE NOTE:
THIS SITE IS FOR THE FILING OF CLAIMS AGAINST THE DEPARTMENT OF TRANSPORTATION THAT OCCURRED ONLY IN DISTRICT 4

District 4 handles the following nine (9) counties: Alameda, Contra Costa, Napa, Marin, San Francisco, San Mateo, Santa Clara, Solano, and Sonoma. Claims not in excess of $5,000.00 that involve incidents occurring within the above counties may be presented to the District 4 Claims Office by completing the claim form below. To request that a claim form be sent to you by mail, please call (510) 286-5806.
